Tidepool and diabetes loops: the data layer behind clinical export and AID workflows

What Tidepool is

Tidepool is a non-profit data platform that aggregates diabetes-device data — CGMs, insulin pumps, glucose meters, and (with appropriate integrations) carbohydrate logs — into a longitudinal record. The platform is best known in the United States but has substantial international presence as well.

For clinicians, Tidepool is the data layer behind the visit-export workflow: a user’s pump and CGM are uploaded, and the clinic reviews the time-in-range, basal patterns, bolus history, and (where logged) carbohydrate intake in a unified view.

For users in the looping community, Tidepool also operates Tidepool Loop, a regulated automated insulin delivery (AID) configuration that has received FDA clearance in the United States. Tidepool Loop is distinct from the older DIY Loop project, although they share lineage.

Tidepool versus HealthKit

HealthKit is a real-time-ish data layer optimized for application-to-application data exchange on a single device. Tidepool is a longitudinal-record data layer optimized for clinical export, retrospective review, and AID configuration.

The two are complementary, not interchangeable:

Carbohydrate-tracking applications and Tidepool

Most consumer carbohydrate-tracking applications do not write directly to Tidepool. The dominant carbohydrate-data path into a Tidepool record is via the user’s pump or AID system bolus history (which records the carbohydrate-input number used in the bolus calculation). For users on multiple daily injection (MDI) regimens without a pump, the carbohydrate log is typically maintained outside Tidepool, in the carbohydrate-tracking application of choice.

This produces a practical limitation: clinical visit reviews based on Tidepool data may not include the carbohydrate-counting application’s logs. Users on MDI who want their carbohydrate logs reviewed alongside their CGM data should bring an export from the carbohydrate-tracking application (PDF, CSV, etc.) to the visit.

Tidepool Loop

Tidepool Loop is the regulated, FDA-cleared AID configuration descended from the DIY Loop project. For users who want the automated-insulin-delivery benefits without the regulatory and support complications of an unregulated DIY configuration, Tidepool Loop is one of the available options (alongside the manufacturer-regulated AID systems from Tandem, Medtronic, Insulet, and others).

Tidepool Loop’s carbohydrate-input is, like all bolus-calculator-based systems, dependent on the accuracy of the user’s carbohydrate count. Users on Tidepool Loop benefit from the same carbohydrate-counting accuracy concerns as users on any other AID system.

Limits

Tidepool is a powerful clinical-data platform. It is not a substitute for clinician interpretation of the data it aggregates. Users should not infer dosing changes from Tidepool exports without the prescribing clinician’s review.
